   I confirm that I have answered the above questions correctly.  

   LIABILITY WAIVER/CONTRACT: I the below signed, do hereby waive any and all claims against any and all persons and groups associated with East Cork Taekwon-Do , I agree to hold harmless, all persons and groups including Irish Taekwon-Do Association, East Cork Taekwon-Do and its representatives, all persons and groups associated with , for any  injuries or damages sustained by me (or my child) in connection with my participation in these classes, whether accidental or intentional.                                       

  I understand and am full aware that I am participating in a contact sport and injuries may occur in training. I am also aware that students are not covered for personal accident cover. 

   Therefore I assume full responsibility for all of my actions during and connected with these classes. I also agree that my attendance and or  performance may be photographed, filmed or taped and used by the promoter and its agents, and waiver any compensation thereof.

This document will form the basis of my contract with the Organizer/Promoter and all its associates/servants/agents.
